Frequently Asked Questions

Dealing with insurance and billing issues can sometimes be confusing. Covenant Heart Specialists' billing department staff will be happy to assist you with your billing questions.

We accept and bill most major insurance plans. We ask that you bring your medical insurance card with you to every appointment to enable us to maintain the correct insurance information.

We do expect payment at the time of service. If we contract with your insurance, we expect your portion of the co-pay, deductible or co-insurance at the time of service. We accept cash, check, Visa, MasterCard, Discover and American Express as payment options.



Q. Will Covenant Heart Specialists bill my health insurance?

A. Covenant Heart Specialists will bill all insurance companies located in the United States. We will not bill for any charges that need to be submitted to an insurance company outside the U.S. but we will provide you with all necessary paperwork so you can submit the claim yourself.

Q. If my insurance was billed, why am I receiving a bill from Covenant Heart Specialists?

A. There can be several reasons you receive a bill:

  • Your insurance company is still processing your claim and we have not received payment.
  • You have changed insurance companies and we are billing your old policy. Please contact us with your updated insurance information.
  • If your insurance has paid on your claim. Covenant Heart Specialists is billing you for the co-pay, deductible or co-insurance which is the patient's responsibility.

Q. Does Covenant Heart Specialists automatically submit billing to my secondary insurance?

A. Covenant Heart Specialists will bill your secondary insurance. Since payment of secondary claims is complicated you will need to monitor your claims to insure proper payment is made from your secondary coverage.

Q. What are the patient's responsibilities in obtaining a referral to see a Covenant Heart Specialist?

A. If your insurance plan requires a referral to see a specialist you must obtain one from your primary care physician and provide us with a copy at the time of your visit.

Q. What are the patient's responsibilities if pre-authorization is needed by their health insurance for a special procedure?

A. Covenant Heart Specialists will obtain authorization for testing ordered by one of our providers. We will assist outside ordering providers in obtaining authorizations for testing they order at one of our clinics. Covenant Heart Specialists recommends that patients contact their insurance plan to confirm that all authorizations are in place before receiving services.

Q. What do I need for my first visit?

A. Please arrive 15 minutes prior to your appointment time to complete the necessary paperwork.

Be sure to bring these items with you:

  • Insurance Cards (Primary and Secondary)
  • Insurance Co-Payment
  • Referral Form from Referring Physician (if required by your insurance)
  • Copy of your medical chart from your primary care physician, as it pertains to your cardiovascular health
  • Medication List with Dosages (Actual Containers Preferred)
  • List of Drug Allergies

During your first visit, your physician will assess your condition and discuss your symptoms and medical history with you. The doctor may then order a variety of tests to further aid in diagnosing your particular condition.
